Community care services can help you overcome those difficulties and maintain or improve your quality of life. The main priority of community care is to give you the support you need in your own home whenever possible. There are many health, housing, and social care services to help you and your main carer.

Community care is for anyone who needs it - for instance, frail older people, people with dementai, carers looking after someone, people with physical disabilities, people with learning disabilities, people with mental health problems, people with substance use problmes and people with HIV and AIDS.
