We aim to promote everyone to have healthy, active lives, maximising your independence.  If it is no longer possible for you to live at home, you may wish to consider Long Term Care.

What happens if I need to move to Long Term Care?

We will undertake an assessment of your needs in discussion with Health professionals and family members to identify the needs and wishes of all concerned.  Following discussion, a variety of options may be available.

Choosing a home

It is important to ensure that the care home meets your assessed needs.  We will work with you and your family to identify three preferred care homes of your choice.  Whilst we will strive to meet your preferences, if none of the care homes are available, an interim arrangement will be made.  However, if you decide you like the interim placement, we can seek for this to become a permanent placement.
