Requiring support after being in hospital?
If you have been in hospital, you may need additional support or assistance to be in place before returning home. We can support you to maintain your independence and safety within your own home prior to leaving hospital.
Our short-term assessment facilities can reduce your length of time in hospital. Our team works with you to assess your abilities and level of independence and establish your care and support needs.
Substance misuse - support and advice
Substance misuse can have a devastating impact on an individual's health, their financial and social circumstances and affect their family, friends and the wider community. If someone is seeking to end their addiction of alcohol, drugs or smoking, there are a number of groups who can offer advice and support to help them quit and offer advice for families who are concerned or affected by the impact of substance abuse.
