Help leaving hospital

Following a hospital admission, people may require additional support or assistance to be put in place prior to returning home.  We work closely with our colleagues in hospitals to ensure that we can offer a variety of services, support and advice so you can safely return home.

How can I arrange for additional support?

Please contact the Ward Nurse or therapy staff to advise that you are seeking additional support on discharge from hospital.  The Ward Nurse or therapy staff will then contact us.  We will liaise with the hospital to arrange appropriate support and will contact the Ward once a care package is available to ensure that you receive support on your return home.

I'm currently receiving the support of carers - how can I arrange for care to restart?

Please contact the Ward Nurse or therapy staff to advise that you are seeking support on discharge from hospital.  The Ward Nurse or therapy staff will then contact us.  We will liaise with the hospital to arrange appropriate support and will contact the Ward once a care package is available to ensure that you receive support on your return home.  Following a hospital admission, you may require additional support on your return home.  The majority of people over the age of 65 will receive our reablement service for a time-limited period on their return home.  This allows us to continually assess you at home to ensure adequate support is in place and you are as independent as possible within your own home.
