Crisis Care
Crisis Care is a short term service which provides care up to a maximum of 72 hours. The service is provided to individuals who, due to a deterioration in ability or breakdown in carer network are unable to continue at home without a support service being provided or an increase in their current service provision. The aim of the service is to enable individuals to achieve maximum support in their own home, to ensure their health and wellbeing as well as their safety.
The service is provided by a team of trained staff from the Intermediate care team. They will provide the necessary support required in the individual's own home.
- A reassessment of the individual will take place within 24 hours of the service commencing.
- If the individual is already in receipt of Care at Home services, they will continue as normal with additional Crisis Care being provided.
- There will be no charge for Crisis Care, but if after the programme the individual is referred for ongoing support, there may be an additional charge for the services received.
Who can receive Crisis Care?
Crisis care is available to people who live within the Stirling Council area who are over 65 years of age and who have a short-term acute illness that has led to a deterioration in ability or have experienced the illness or loss of an informal carer. People are also required to be registered with a local General Practitioner who agrees to continue medical care.
How can I access Crisis Care?
In order to access Crisis Care, a referral is required from a health professional, through a care assessment or through our out of hours emergency service.
