Reablement carersReablement is a care at home service to help older people enjoy living safely and independently in their own homes for as long as possible. 

It is all about giving you the encouragement, support, skills and confidence you need to stay in your own home, or to be able to return home after a stay in hospital.

The focus is on working together to assess what you can do - and what we can help you learn to do again.

The service is for people aged 65 and over who live in the Stirling Council area.  Reablement can help when someone has had a fall, illness, stay in hospital, or find that they can't manage without support.

How does reablement work?

Reablement front pageThe reablement team will work with you for a number of weeks, providing support and encouragement to help you get back up to speed.  The team will include people with the skills and experience to help you regain as much of your independence as possible.

The support we provide is practical.  For example, we can arrange for you to have equipment fitted in your home to help you with everyday tasks.   Staff can also put you in touch with other services that might be required to help you keep safe and well.  Reablement is provided between 7am and 10pm. 

You will be part of all the decisions about your reablement, and you'll work with the reablement team to set your own goals.  We'll discuss your progress with you regularly.

How do I access reablement?

Before beginning reablement, an assessment of your needs and abilities is required.  Assessments can be from community or hospital-based health professionals, or via Community care assessment

How long do I receive reablement?

Reablement is part of an ongoing assessment process which lasts up to 6 weeks.  Reablement is a way of making sure you get the right care to meet your needs.  Once the reablement plan has been completed:

  • you may no longer need support and can continue to live independently

or

  • you may need ongoing support

For people who need ongoing support, we have made arrangements with three care companies to provide home care services. These companies have been chosen because they have long records of providing high quality care.  Their work will be checked by Stirling Council and the Care Inspectorate (SCSWIS). 

Alternatively, you may be interested in receiving Direct Payments, which provides you with an agreed amount of money to purchase your own care and support, meaning more choice and control about how your needs are met, who helps you and when they help you.  Further information about Direct Payments.

Do I pay for Reablement?

Reablement is free of charge while you take part in the service.  If you require ongoing support needs after reablement,the charge will be in line with our Social Care Charing Policy.
