Rehabilitaton at home
Rehabilitation at Home aims to provide time-limited support, to a maximum of 4 weeks, to individuals to enable achievement of maximum independence and to prevent unnecessary admission/re-admission to hospital.
Who can receive Rehabilitation at home?
Rehabilitation at home is available to people who live within the Stirling Council area who are over 65 years of age and who have either a short-term acute illness that has led to a deterioration in ability, experienced the illness or loss of an informal carer, have an exacerbation of a chronic condition, or experienced an acute trauma which has resulted in a deterioration of physical function. People are also required to be registered with a local General Practitioner who agrees to continue medical care, be medically stable for 48 hours prior to referral with potential for Rehabilitation and be able to transfer independently.
How can I access Rehabilitation at home?
In order to access Rehabilitation at home, a referral is required from a health professional, through a care assessment, or through our out of hours emergency service.
