Care - home assessment
If you are you facing difficulties with:
- ill health,
- disability
- the effects of getting older
- or, you are caring for someone
Community care can help you overcome difficulties and maintain or improve your quality of life. The main priority of care is to give you the support you need in your own home whenever possible. There are many health, housing, and social care services to help you and your main carer.
Community care is for anyone who needs it -examples are: people who are
- frail and or elderly
- suffering from dementia
- living with a physical disabilities
- with learning disabilities
- suffering from mental health problems
- dealing with substance abuse problems
- and who have contracted HIV and/or AIDS.
In order to access services that meet your needs, we will undertake a Community care assessment.

There are a variety of ways to access services and there may be a charge for some services.
If services are provided by other agencies or the voluntary sector, please contact them in the first instance if you need more information.
