Consent for activities and excursions

Use this online service to let us know you consent to excursions with the youth participation team.

This information is being requested to keep the young person safe while they take part in activities. It will cover any excursions for 12 months from the date it is submitted. We will notify parents or guardians of any excursions by letter, email or text. You can opt out of that excursion by contacting the team.

Who should complete this form?

• If the young person is age 12 to 15 – the parent or carer should complete the form on their behalf. The parent should discuss and agree an approach to the use of visual imagery and personal data with the young person before submission.
• If the young person is 16 or over – the young person should complete the form
