General claim

Use this form to make a claim that's not about roads, footways or housing, such as damage to personal property or personal injury.

Before you begin

Make sure you're ready to provide details of:

  • the exact location and time of the incident
  • information on any witnesses
  • your date of birth
  • information on your vehicle, if this relevant
  • details of any injury that you sustained
  • details of any treating doctor or NHS hospital, if that applies

If you have them, you should also be ready to upload:

  • relevant photographs
  • supporting documents, such as invoices or repair estimates

If you need to, gather all of this information before you begin. You will not be able to save your progress once you start filling in the form.
